Critical Nurse Staffing, LLC is seeking a Case Manager RN to join our team, providing quality care to our patients in New Mexico. Must be based out of New Mexico.

POSITION SUMMARY: This position is responsible for the coordination and delivery of nursing care for assigned patients served by this agency and for the day-to-day management and supervision of the related clinical care activities within the area the Case Manager services.This position will assume responsibility for implementing the Plan of Care for patients assigned. This position is also responsible for supervising and responsibly directing the activities of various levels of assigned care and ancillary personnel utilizing both supervisory discretion and independent judgment.
KNOWLEDGE, SKILLS AND ABILITY:
Utilizes interpersonal skills.
Interacts in an emotionally supportive way with the client, their family and caregivers.
Observes, reports and documents changes in client status.
Recognizes emergency situations and implements appropriate emergency procedures per agency policy.
Follows agency procedures with regard to infection control, handling of hazardous wastes, and safety measures.
Accurately documents care provided and submits notes and other required documents per agency policy.
Has basic computer efficiency in: Word, E-mail, company internet and intranet, other electronic systems as may be required to perform Agency and medical record, communication and information systems functions.
Position Specific:
Ability to set priorities appropriately.
Bases decisions on fact and good judgment; seeks assistance when necessary.
Ability to travel as needed.
Ability to effectively supervise others.
Qualifications for this position include; a degree in nursing (B.N. or B.S.N preferred) and a current and valid Washington state RN license. Current ability to supervise in each state where patient care is supervised.
Current and valid driverâs license and proof of insurance issued from the state in which the employee resides. The license must be without limitations or conditions, and employee must be able to provide driving record of all violations within the last year.
Preferred qualifications include;
Recent experience in home care.
Minimum one (1) year of home care supervision.
