Bachelor’s Degree in Nursing or healthcare related field, or with the equiv! alent years of experience in hospice management required. 3-5 years clinical experience in direct patient care, with at least one year in home health/hospice with case management oversight experience. 3-5 years experience in supervision and/or management required. NM Registered Nurse License required. Strongly prefer knowledge of clinical case management. Prefer facility in use of common PC software such as MS Word and Excel.

Primary Job Functions

  • Acts as Clinical Director in absence of Hospice Director; acts as Clinical Resource for Department.
  • Hires, orients and supervises clinical staff to include monitoring individual performance, productivity, and quality of services provided. Directs or performs the annual evaluation of caregiver staff performance, including supervised field visits. Identifies training and development needs, and makes recommendations for promotion.
  • Hires, orients and supervises Hospice staff to include moni! toring individual performance, productivity, and quality of wo! rk.
  • Monitors all daily clinical operations, interprets and educates staff in compliance with to state and federal regulations, internal policies, and accreditation standards.
  • In collaboration with peers and Hospice Director participates in clinical services program development and evaluation to ensure an effective patient care delivery system.
  • Meets regularly with staff to consult regarding case management, use of service, and documentation of care.
  • Proactively manages the relationship between service demands and availability of staff. Assures that patient needs are identified and met, and that qualified caregivers are assigned consistently.
  • Provides counsel in the interpretation of nursing standards and company policies and procedures to all levels of caregiver staff.
  • Directs development and implementation of an outcome oriented patient care plan that conforms to the physician’s plan of treatment. Ensures that communi! ty and agency resources are utilized to manage the care plan and assure that stated outcomes are achieved. Periodically reassesses patient needs and progress toward goals.
  • Directs caregiver staff in the clinical record documentation process to ensure that the patient’s clinical record meets legal and regulatory requirements, facilitates care, enhances the continuity of care, helps coordinate treatment and evaluation of the patient, and establishes medical necessity. Performs start of care, ongoing care, and discharge record audit.
  • Conducts team conferences on all multi-disciplinary, hi-tech or problem cases as needed but not less than every 2 weeks. Team conferences will also be conducted on routine cases to ensure adequate coordination and quality of care.
  • Ensure that all patients receive appropriate care 24 hours a day, 7 days a week which demands close detailed interaction, reporting, and collaboration with after-hours staff.
  • Interac! ts with third party representatives, such as case managers and payers, ! to obtain needed re-authorizations and negotiate for additional services and supplies for the patient after admission.
  • Reviews and monitors census, length of stay, productivity and adherence to budgetary goals. Collects statistics and provides reports as requested. Utilizes reports and information system to enhance the delivery and recording of care at PHH.
  • Responsible for maintaining a positive relationship with referral sources and community resources. Contributes to the growth and expansion of PHH services through delivery of Customer Service, patient advocacy, education of referral sources and physicians, referrals to other products within PHH, and excellent communication with all related parties.
  • Provides responsive, customer focused resolution to patient care issues, concerns and conflicts. Informs Hospice Director of significant customer service complaints and issues.
  • Participates in Quality Assessment and Improvement activities an! d assures participation of all appropriate staff. Provides information that enables the collection and root cause analysis of data to identify opportunities for improvement.
  • Prepares draft goals and budget for management area.
  • Participates in professional organizations.
  • Rotates PHH clinical management pager.
  • Performs other functions as required.
