Part-Time Care Provider

Location: Chicago Illinois

Basic Function: To provide residents who live in Esperanza Community Services’ CILA homes (Community Integrated Living Arrangements) the services needed to attain their fullest measure of growth in the least restrictive environment possible. Reports to CILA Program Director.

Duties:

  • Serves as member of the Community Support Team (CST).
  • Assists residents with goals and objectives stated on their Individual Service Plans (ISP).
  • Assists residents in obtaining services as stated on ISP’s.
  • Transpor! ts residents in CILA van or car as needed to enable their part! icipation in activities and appointments outside the CILA.
  • Documents the implementation of the ISP goals, objectives, and service objectives.
  • Maintains records of events on direct care shifts and other records as needed.
  • Communicates with the Program Director, Assistant Program Director, QIDP and other care and service providers as necessary for clients’ well-being.
  • Attends all staffings as scheduled by the Program Director, Assistant Program Director and/or QIDP.
  • Uses special talents to aid the residents in the development of their own skills in leisure and recreation.
  • Works with residents and CST members to keep the CILA and its grounds clean and to do other household/hygiene chores as necessary.
  • Attends all seminars and fulfills required tests and certifications as required by the Illinois Department of Human Services.
  • Works with volunteers, interns and other clients’ well-being as assigned by Program, A! ssistant Program Director or QIDP.
  • Oversee the client’s self-monitoring of medication after training by nurse.
  • Fulfills other duties that may arise in order to enhance the quality of life and wellbeing of CILA residents and the effectiveness of the CST.

Qualifications: High School Diploma or GED, minimum. Valid Illinois driver’s license and proof of auto insurance required. Training as required by DHS rule 115.
