RN Case manager is responsible for evaluating assigned patients to determine their health care barriers through a variety of assessments to identify areas of concern (financial, environmental, psycho-social, caregiving) and potential gaps in care.
There is no medication dispensing with this role. This NOT a home care position but a managed care delivery.
Based on areas of concern, identify appropriate interventions that may include education, community based resources, or program/agency referrals.
With the patient and caregiver (as appropriate), develop and coordinate an action plan to implement the interventions.
Act as the patient’s advocate and liaison by completing or facilitating face to face or telephonic interventions with providers and/or private, non-profit, and governmental agencies.
Work collaboratively with the entire care team to ensure knowledge of the action plan in support of the patient care plan, to include participation in case conferences.
Document and maintain assessment and intervention data in appropriate applications and platforms.
Comply with all performance standards and reporting requirements.
Maintain an understanding of ! the clinical program design, implementation, and management, m! onitoring, and reporting.
Experience:
Registered Nurse (RN) License in Connecticut
5 + years RN experience
Prior Care Planning experience
Home Care or Hospital Assessment background
Excellent documentation skills using electronic medical records technology systems
Patient advocate
