Duties and Responsibilities:
The Registered Nurse (RN) will be a qualified RN who meets the legal requirements to practice as a professional RN in California. Under the Supervision of the Supervising Registered Nurse (SRN) the RN will be responsible for duties in the Skilled Nursing Facility (SNF) including but not limited to the following:

Make rounds with physician and report any unusual resident conditions to physicians and SRN without delay. Transcribe/notes and physician’s orders. Gives medication and treatments and record information in proper areas. Assume responsibility for seeing that consents, laboratory reports, and charts are completed.

Complete assigned weekly summaries as scheduled. Documentation in these summaries will reflect the resident care plan. Documentation of resident care will be completed as scheduled.

Interview each resident on admission and explain rol! e of various staff members on the unit. Do nursing history and assessme! nt, complete physical assessment; document findings completely, including skin assessment. Identify nursing problems including physical, emotional and social needs and begin formulating a written plan of care. Orient resident to physical layout of unit, specific unit routine and use of call bell system. Assure that physical inventory of residents’ belongings is completed and clothing and personal items are marked with resident’s name.

Formulate a written nursing care plan for each resident in consultation with all unit nursing staff and physician. Review plans weekly and as indicated by change in resident condition. Observe residents frequently, plan appropriate nursing care and document pertinent information in nurses’ notes. Identify rehabilitation potential of individual residents and plan care directed toward attaining optimum level of function as demonstrated by maintaining proper body positioning and body alignment of bedfast residents and maintaining a! dequate nutritional fluid intake.

Assist in other units as need arises.
