Job Duties:
The Hospice Social Worker plays a key role in assisting the patient and family during their journey at the end of life. The hospice Social Worker visits our patients, primarily living in the nursing home environment, and collaborates with the patient and family to promote the psychosocial well being and address coping issues that may arise. In the role, you will also assist in coordination issues regarding financial assistance and funeral home procurement. To achieve these goals the hospice Social Worker is intimately involved in a collaborative relationship with the patient, family and the entire interdisciplinary hospice team. This role allows you to utilize your clinical skills while you develop meaningful relationships with your clients, their families, and with other members of our extraordinary team. You will be empow! ered to perform excellence in care through realistic ratios en! abling you to spend quality time with patients. Our Social Workers are field based positions.
Job Requirements
The ideal candidate will have 1 to 2 years of experience woking in healthcare and be willing to travel in their local area. The ideal candidate will also possess exceptional clinical skills, a devotion to patient centered care, and a warm engaging interpersonal style. Additionally:
masters of Social Work is required
Prior hospice experience is a plus
A reliable automobile
Current driver's license (mileage is generously reimbursed)
Current car insurance
Computer proficiency
Ability to multi-task and be highly flexible
Ability to work independently, yet highly collaboratively as part of a team
