Description
Under the supervision of the Director of Case Management, the Social Worker Assists in meeting ! the psychosocial needs of patients and enabling patients to utilize medical care in order to achieve their optimal level of health. Assesses patients for discharge planning and assists with referrals to appropriate local/state/ community resources. Educates patients, families, or significant other regarding options for care outside the hospital as appropriate including but not limited to nursing home placement, home health, hospice, rehab, long term acute care, assisted living and DME needs. Assists with filing applications for financial aid. Assists in adoption of infants on the Women's and Children's unit per organizational policy when request received. Works closely with victims of abuse and/or neglect, makes appropriate referrals to the Department of Human Resources and provides appropriate resources per departmental policy. Assists with DHR guardianship when needed.
Qualifications
Education:
Required: Graduation from an accredited sc! hool of Social Work with a minimum of a Master’s Degree in S! ocial Services.
Experience:
Required
:
A minimum of two years professional experience in a clinically related area. Excellent oral and written communication skills. Continuing education courses/competency validation in areas which update knowledge and expertise in discharge planning, financial assistance and community resources required. Knowledge of Social Work Theory and Practice required.
Preferred
:
Previous experience in an acute care setting in Case Management. Familiar with local referral agencies. Working knowledge of Medicare, Medicaid and other third party payers preferred.
Licenses/Certificates:
Required: Current licensure in the State of New Mexico in Social Services.
