As a Home Care Case Manager - Registered Nurse (RN) you will be responsible for providing nursing care to clients and directio! n and leadership to designated staff.
Additional responsibilities of the Home Care Case Manager - Registered Nurse (RN) include:
- Assessing and formulating a plan of care for clients in their homes that includes social, emotional, physical, and spiritual needs.
- Providing nursing care to clients according to physician orders and regulations.
- Providing teaching and training to clients, families, and caregivers as appropriate.

To qualify for the Home Care Case Manager - Registered Nurse (RN), you must have effective communication skills and be able to work with individuals of all ages.
Additional requirements of the Home Care Case Manager - Registered Nurse (RN) include:
- Graduate of an accredited school of nursing.
- Licensure for state of practice is required.
- CPR Certification or ability to achie! ve upon employment.
- Must be at least 18 years of age.!
- Must have a favorable motor vehicle record (MVR) and must present a valid drivers license and proof of insurance before operating a vehicle.
SPECIFICATIONS: This position is only an on-call position, every weekend from Friday at 4:30pm to Monday at 8:00am.
Employee is paid $2.00/hr for on-call hours, and hourly rate for work performed, plus travel time and mileage.
SUMMARY OF DUTIES: Provides in-home nursing care to assigned clients, in a caring, safe and efficient manner. Assures medications and treatments are administered to assigned clients. Demonstrates familiarity with the therapeutic effects and side effects of all medications ordered. Correlates medications to diagnosis. Accurately completes glucometer testing and control monitoring according to procedure.
Assures physicians’ orders are processed and physicians notified in a timely manner. Directs and documents care pro! vided to clients. Utilizes appropriate diagnosis codes that are supported by documentation. Updates in a client’s diagnosis and any resulting change in codes shall be communicated to the billing office in a timely manner. Performs accurate data entry. Receives and shares information; observes and reports clients’ conditions to appropriate staff. Explains procedures before and during care. Directs and assists with client, family / significant other education.
JOB REQUIREMENTS: Must have current RN licensure. Two years nursing experience, preferably in home health. Must have valid driver’s license with an acceptable driving record, reliable transportation, current automobile insurance. Bilingual a plus. Current CPR certification or ability to become certified within 180 days of hire. Cannot have any qualifying convictions.
WORK LOCATION: Work is performed primarily in the client’s home.
WAGE: $23.50 to $25.32/hr (dependi! ng on experience).
