HOME HEALTH RN
The Registered Nurse provides skilled nursing care to patients in their home under the general guidance and direction of the Case Manager, according to the physician's orders and in accordance with Harmony Home Health policies and procedures, and following established standards of care. Essential Duties and Responsibilities:
Provide those services and/or treatments requiring substantial and specialized nursing skill.
Function as a team membe! r demonstrating collaboration with, and responsibility to pati! ent, physician, and interdisciplinary team.
Perform nursing care according to the needs of the patient and as indicated in the written plan of care. Assist in the assessment of needs, formulation and review of patient care plans..
Have a good understand of Medicare guidelines.

Qualifications and Experience
Graduate of an accredited school of nursing. (Diploma or transcripts to be provided to agency)
Currently licensed in the Operating State as a Registered Nurse.
One (1) year nursing experience recommended, experience in home health care recommended.
Knowledge of regulations and procedures to administer patient care.
A current TB test, CPR card, driver’s license, automobile insurance must be kept on file.
Must have and maintain a reliable automobile.
A criminal background check ! must be performed before hire.
