Case Manager

Case Manager - Home Healthcare (Albuquerque) medical, incentives, paid time off (PTO)compensation: $18 to $22 per hour plus incentive pay.
With incentives the compensation is over 50K annually.
Home Health Care Comp any hiring a Case Manager .
* work with the families of patients to insure all needs are fulfilled.
* work with the Field Nurses to coordinate care.
* work with each individual case to manage goals towards the prescribed plan of care.
* communicate back to treating physicians' offices to keep them informed of patient status.
* periodically attend Doctor's appointments with patients.
This is a full time position in an office setting. Will be working with good experienced people. Need to be an excellent and articulate medical writer, communicator, problem solver, able to multi task, proficient on the computer (good Excel, Word skills required). This position brings great variety of tasks. You will be reviewing charts for concerns, occasionally making patient visits, going to doctor appointments, coordinating DME, training of field nurses and home health aides on company policy and procedures, preparing correspondence with physicians, w orking side by side with case managers on a daily basis to ensure coordination of care is meeting the needs of the patients and more.

We're looking for that certain warm person that wants to take care of patients and work with a great team in the process. Work is similar to medical office administration and clerical. This is a full time position, 40 hours a week. Pay will be based on experience, education, and capacity. Office is located near I-40 and Coors interchange.
